What Is Sinus Tarsi Syndrome?

Sinus tarsi syndrome is a clinical disorder of persistent pain and tenderness localised to the sinus tarsi — a small cone-shaped canal on the outer (lateral) side of the hindfoot, sitting between the talus (ankle bone) and the calcaneus (heel bone) at the level of the subtalar joint.

Within this tunnel lie important ligaments (the cervical and interosseous ligaments), nerves, fat and blood vessels that contribute to proprioception and subtalar joint stability. When these structures become inflamed or scarred — most commonly following an ankle sprain — the resulting irritation produces deep, aching pain and a characteristic feeling of ankle instability.

Sinus tarsi syndrome is frequently confused with a chronic lateral ankle sprain or general ankle pain, and patients often wait months before receiving an accurate diagnosis. With the correct clinical assessment and treatment it typically responds well to conservative management.

The Sinus Tarsi & Its Role

The sinus tarsi forms the anterolateral opening of the tarsal canal, running obliquely between the talus above and the calcaneus below. It acts as an important stabilising structure for the subtalar joint, which controls the side-to-side movement of the foot during walking and running.

The interosseous and cervical ligaments within the sinus tarsi are the primary restraints to excessive subtalar inversion. When these ligaments are injured — typically in an inversion ankle sprain — they can become inflamed, develop scar tissue, or lose their proprioceptive function, leading to persistent pain and instability.

The Classic Presentation

The hallmark of sinus tarsi syndrome is persistent lateral ankle pain following an ankle sprain that appears to have largely healed. The initial swelling and bruising settle, but a deep, nagging ache and sense of instability remain — often for months — because the underlying sinus tarsi inflammation or ligament scarring has not been addressed.

Symptoms are commonly worse on slopes, stairs or uneven terrain. Many patients describe a loss of confidence in the ankle, feeling it might "give way" even when the ligaments themselves are intact. A diagnostic injection of local anaesthetic into the sinus tarsi that abolishes pain confirms the diagnosis.

Differential diagnosis
Conditions That Can Feel Similar

Lateral ankle and hindfoot pain is common to several conditions. Accurate diagnosis before treatment is essential — particularly as sinus tarsi syndrome is so frequently assumed to be a simple ankle sprain.

Lateral Ankle Ligament Sprain
Acute ligament injury produces swelling, bruising and instability around the lateral malleolus. The key distinction is temporal — sinus tarsi syndrome presents as pain that persists or re-emerges after the sprain has apparently healed, often with the pain sitting slightly forward of the ankle bone rather than directly over the ligaments.
Peroneal Tendinopathy
Pain and tenderness running along the peroneal tendons behind and below the lateral malleolus. Unlike sinus tarsi syndrome, pain follows the tendon line and is reproduced by resisted eversion and single-leg heel raises. The two conditions can coexist and should both be assessed after ankle sprain.
Subtalar Joint Arthritis
Degenerative change in the subtalar joint produces hindfoot pain and stiffness similar in location to sinus tarsi syndrome. Distinguished by a more gradual onset unrelated to ankle sprain, global stiffness of the subtalar joint on passive testing, and arthritic changes visible on X-ray or MRI.
Cuboid Syndrome
Subluxation of the cuboid produces lateral mid-foot pain that can be confused with sinus tarsi syndrome. Cuboid syndrome pain tends to sit slightly further forward along the outer foot, is reproduced by specific cuboid provocation tests, and responds to cuboid manipulation — distinguishing it from the deep ankle pain of sinus tarsi syndrome.

Weakness of the peroneal muscles is common after ankle sprains and compromises lateral ankle stability, increasing stress on the sinus tarsi. A progressive strengthening programme targeting peroneus longus and brevis, alongside calf and intrinsic foot muscle work, supports the subtalar joint and reduces the risk of recurrence.

Taping & Bracing
In the acute or sub-acute phase, low-Dye taping or a lateral ankle brace can limit subtalar inversion, reduce sinus tarsi compression and allow pain-free activity during rehabilitation. Is sinus tarsi syndrome the same as an ankle sprain?
No — though it most commonly develops after one. An ankle sprain is an acute ligament injury that typically heals within weeks. Sinus tarsi syndrome is a specific clinical condition involving inflammation or scarring within the sinus tarsi canal that persists after the sprain has healed, producing ongoing pain and instability. The pain sits slightly forward of and below the lateral malleolus, rather than directly over the ankle ligaments.
Will my X-ray or MRI show anything?
X-rays are usually normal. MRI is the most useful imaging investigation and may show fluid, synovitis, fibrosis or ligament signal changes within the sinus tarsi — but even MRI can appear relatively normal in clinically confirmed cases. Diagnosis is primarily clinical, and a diagnostic local anaesthetic injection that abolishes pain is often the most reliable confirmation of the diagnosis.
How quickly does a corticosteroid injection work?
Most patients notice a significant reduction in pain within 3–7 days of the injection, with the full anti-inflammatory effect felt at around 2 weeks. This window of pain relief is important — it allows rehabilitation and proprioceptive retraining to begin without the barrier of constant pain. The injection alone is rarely sufficient long-term; it works best as part of a broader treatment plan.
Can flat feet cause sinus tarsi syndrome without a sprain?
Yes — excessive pronation compresses the soft tissues within the sinus tarsi canal with every step, and over time this can cause chronic irritation and sinus tarsi syndrome without any single traumatic event. This presentation tends to have a more gradual onset and is particularly common in runners who have increased their training load. Orthotics addressing the pronation pattern are a central part of treatment in these cases.
How long does recovery take?
For recent-onset cases with a clear cause, symptoms often improve significantly within 4–8 weeks with the right treatment. Long-standing or chronic cases — particularly those that have been mismanaged as recurrent ankle sprains — may take 3–6 months to fully resolve. Early accurate diagnosis makes a significant difference to recovery time.
