Metatarsalgia & Forefoot Pain
Pain in the ball of the foot is rarely just soreness — it often signals a specific overload pattern that needs identifying. Accurate diagnosis determines the right treatment and gets you back on your feet faster.
Metatarsalgia is a broad term describing pain in the ball of the foot — the area across the metatarsal heads beneath the toes. It is not a single condition but a symptom pattern that can arise from several distinct causes, each requiring a different treatment approach.
The metatarsals bear significant load during walking and running — particularly during push-off, when nearly the entire body weight passes through the forefoot. When load is unevenly distributed or a specific structure is overloaded, pain develops and typically worsens with activity and time on feet.
Several conditions can present as metatarsalgia. Accurate diagnosis is essential — treatment varies significantly depending on the cause.
Mechanical Overload
Excessive pressure through one or more metatarsal heads — often from a dropped metatarsal, altered foot posture or high-impact activity.
Morton's Neuroma
Nerve thickening between the metatarsal heads causing burning, shooting or numb pain — most commonly between the 3rd and 4th toes.
Intermetatarsal Bursitis
Inflammation of the bursa between the metatarsal heads, producing aching forefoot pain that can mimic neuroma.
Metatarsal Stress Fracture
A stress fracture in one of the metatarsal bones — common in runners — producing localised bony tenderness and swelling.
Sesamoiditis
Inflammation or injury to the sesamoid bones under the first metatarsal head — causes pain directly beneath the big toe joint.
Footwear & High Heels
Narrow toe boxes and elevated heels concentrate load through the metatarsal heads and are a major driver of forefoot pain in women.
Aching, burning or sharp pain across the ball of the foot
Pain that worsens with standing, walking or running
A sensation of walking on a pebble or bunched sock
Numbness or tingling into the toes
Localised swelling or tenderness over a specific metatarsal
Relief when removing shoes and resting the foot
Diagnosing the cause of forefoot pain requires a detailed clinical examination. We assess the location and nature of tenderness, test specific structures, and evaluate foot posture, footwear, and gait. Where a stress fracture is suspected or the diagnosis is unclear, we arrange imaging referral — X-ray for bony pathology, ultrasound or MRI for soft tissue causes.
Treatment is tailored to the specific cause. Most forefoot pain responds well to conservative management when the correct diagnosis is made and load is appropriately managed.

Reduce heel height — even a small reduction significantly lowers forefoot load

Use a metatarsal pad in your shoes to offload the painful area while awaiting assessment
If forefoot pain has persisted for more than 2–3 weeks, is limiting activity, or is worsening, an assessment is recommended. The wide range of conditions that can cause forefoot pain means self-treating the wrong diagnosis often delays recovery significantly.
If you have sudden onset localised bony pain after increasing activity, a stress fracture should be ruled out promptly — these require a period of offloading to heal correctly.
What is the difference between metatarsalgia and a neuroma?
Metatarsalgia is a general term for forefoot pain. A neuroma is a specific cause — nerve thickening between the metatarsal heads — that produces burning, shooting or numb pain. Metatarsalgia can have many causes; a neuroma is one of them. Clinical examination distinguishes between them.
Do I need a scan?
Not always. Most forefoot pain is diagnosed clinically. Imaging is arranged where a stress fracture is suspected, the diagnosis is unclear, or symptoms are not improving with treatment.
Can I keep running?
Depends on the cause. Stress fractures require a period of rest from impact. Mechanical overload and neuroma can often be managed with load modification and footwear changes while continuing training at a reduced level.
Will orthotics help forefoot pain?
For mechanical overload and pressure-related forefoot pain, metatarsal padding and orthotics are highly effective. For neuroma, they work best combined with footwear changes. For stress fractures, offloading footwear or a walking boot is required first.
